GRINDlab requires EVERY visitor to our park to have a completed, signed WAIVER, RELEASE and ASSUMPTION OF RISK AGREEMENT.
We cannot make any exceptions to this rule.
If you are over 18 years of age.
- Download and Print the WAIVER
- Fill Out and Sign
- Bring it with you when you come to our park
If you are under 18 years of age, you MUST:
- Bring a parent with you to sign in the presence of a GRINDlab Employee, or...
- Download the WAIVER form, and have a parent sign it in the presence of a NOTARY, and have it NOTARIZED. Forms signed by parents without a GRINDlab employee as a witness cannot be accepted. Forms that do not have the NOTARIZED STAMP/SEAL will not be accepted either.
